Documents to Carry
- Self-Attested Birth Certificate of the child.
Photocopy of the Aadhar Card of both Parents and the Child.
Photocopy of Father’s caste certificate. (If you belong to SC/ST/OBC)
- Residential Proof (Electricity bill/gas bill)
Attested copy of the leaving certificate of both the parents.
Photocopy of Child’s Bank A/C Passbook. (With IFSC Code)
Postcard size photo with white background of the child with her/his Father and Mother. The Child’s full name and address must be written behind the photograph in block letters.
One long white envelope with the name of the applicant, address and mobile no. written on it.
